ABSTRACT

Purpose: The purpose of this study was to evaluate clinical outcomes and bone changes using Cone-Beam Computed Tomography (CBCT) before and after IR for periodontally compromised teeth.

Methods: Fourteen periodontally involved teeth were selected for intentional replantation (IR). The preoperative orthodontic procedure was performed to apply luxation for atraumatic extraction to teeth. The tooth was gently extracted. After removing the granulation tissue and necrotic cementum on the tooth root surfaces, retrograde filling of the root-end canal was performed with mineral trioxide aggregate (MTA) or 4-META/MMA-TBB dentin-bonded resin. Once tooth was repositioned in the socket, it was splinted with the adjoining tooth. After 3 months, prosthetic restoration was performed. Clinical parameters such as pain, probing depth (PD), bleeding on probing (BOP) and mobility of tooth were measured. CBCT images were taken twice before and after IR surgery. Heights of alveolar bone were measured using software based on CBCT images.

Results: No clinical complications occurred in any of the cases. In this study, most preoperative symptoms, including pain, mobility, probing depth, and bleeding on probing, were significantly decreased (pain: 4.71 to 1.00; mobility: 1.36 to 0.29; PD: 5.60 to 2.85; BOP: 3.50 to 0.79). There have been no occurrences of generalized root resorption, no evidence of periapical rarefaction based on the radiographic evaluation. Bone height were increased after IR according to CBCT analysis (amount of change: maxilla: 4.00; mandible: 1.95). No significant differences in bone height were observed.

Conclusions: In this study, IR of periodontally compromised teeth showed favorable results in clinical and radiographical evaluation. It suggests that IR seems to be an alternative therapy for extraction in tooth with periodontal disease.
